We are proud to announce that we offer Quality Dental Plan in our dental office. Quality Dental Plan is an annual dental savings program we offer for individuals that allows all patients to receive quality dental services at greatly reduced prices in our office. Unlike conventional insurance plans, with our Quality Dental Plan program there are no deductibles, no yearly maximums, and no waiting periods to begin treatment. Quality Dental Plan benefits and savings begin immediately upon registration.
Our Quality Plan includes:
• INCLUDED routine teeth cleanings benefits (up to 2 per year)
• INCLUDED scheduled dental examinations (up to two per year)
• INCLUDED all recommended x-rays
• INCLUDED Complimentary teeth whitening (value of $60)
• INCLUDED emergency exam during normal business hours (up to 2 per year)
• INCLUDED topical fluoride if recommended (up to 2 per year, 18 and younger)

Added Benefits:
• A 12% savings on general and major dental procedures
• A 12% savings on implants and implant restorations

The fee paid for our Quality Dental Plan is for included standard of care services and represents a courtesy accounting adjustment for payment, made in full, at the time of service. An initial fee is $456.00 for one individual. Fees are due and payable when services are rendered and are non-refundable when services have been provided. Savings duration is for one year from registration date. All patient portions for services received are due at the time of services in order to receive the programs savings.
Interest-free payment options may be provided in 3, 6 or 12-month duration are available upon request and with approved credit through Care Credit for dental services other than the Quality Dental Plan fee.
If an appointment is missed without giving our office proper notice, a fee will be charged depending on the amount of time scheduled and will not be less than $45. Of course, we understand if an unexpected emergency occurs. Please notify our office at least 2 business days in advance if you must change your reserved appointment

What is Scaling and Root Planing?
Scaling and Root Planing is a deep cleaning below the gumline used to treat gum disease. Gum disease is caused by a sticky film of bacteria called plaque.

• INCLUDED: Scaling and root planing (up to 4 FULL quadrants)
• INCLUDED: scheduled dental examinations (up to 2 per year)

• INCLUDED: emergency exam during normal business hours (up to 2 per year)
• INCLUDED: topical fluoride if recommended (up to 2 per year, 18 and younger)
